2009 Nomination Forms

Corporation (More than 99 Employees)

Complete all requested information on the appropriate nomination form and save the completed form. If sending by email please send the completed form as an attachement. Unfortunately, incomplete nominations will not be reviewed.

Submit the nomination form and any supporting documents (e.g., news clippings, resumes, photographs, etc.) to Serve Idaho via e-mail to Peggy Blodgett (preferred), fax (208-327-7470) or U. S. Mail (317 West Main Street, Boise, Idaho 83735).  Applications must be received by close of business on October 12, 2009.  Please submit no more than four (4) pages of supporting documents and include the nominee’s name on the top right of each page.
